When you have been diagnosed with an incurable illness it is only natural to think about the future with a sense of uncertainty. An advance care plan allows you the opportunity to think about what is important to you and your family.
What is an advance care plan?
It can help you to identify what medical treatments you wish to have or not have if you were not in a position to decide for yourself. It helps the medical team keep your best interests in mind and also allows your family to know your wishes if you are not in a position to tell them.
What does it involve?
An advance care plan involves input from both you and your health care team and can be altered or changed at any time. You can talk to your doctor or nurse if you would like some help or guidance to start an advance care plan.
